We provide a comprehensive array of treatment services and also offer a number of specialized programs designed to prevent mental and emotional problems or to provide ongoing support and assistance to persons with more severe forms of mental illness. Some of our services include:

Adult Case ManagemenT
|
Adult Case Management, also known as Community Psychiatric Supportive Treatment services (CPST), is a collection of services that can assist you and your family. We focus on assisting you in identifying and accessing services such as medical, psychiatric, social, financial, and educational. CPST services are usually long term, ongoing, and designed to make sure that you receive the kind of care and service you need in order to live a more satisfying life.
Case Management
What Are Adult Case Management Services?
Adult Case Management services are a community-based service, typically delivered in one’s own home or in the community. Case Management is individualized, recovery-oriented and focused on enhancing an individual’s ability to succeed in the community and minimize barriers to treatment.
What Can I Expect from My Case Manager?
At the onset of services, your case manager will conduct a thorough assessment to understand your needs, strengths and challenges to live independently. Based on this information, you both will work together to develop a comprehensive individualized service plan and develop goals for treatment. This is an ongoing plan that can be updated at anytime to show progress or changes in goals.
Your case manager will focus on different facets of life including but not limited to:
- Linking to community resources and social support
- Assistance with navigating resources for transportation, safe housing, medical assistance, medication management, and more.
- Guidance and education on performing daily tasks, such as budgeting, personal care, meal planning and other identified needs.
- Collaborate with community treatment providers to ensure continuity of care.
Frequency of Case Management Services
Case Management services are individualized, and frequency will be determined through medical necessity. You and your case manager will work to establish how often to meet. Some months may vary based on individual need. Case Management can be short-term or result in more long-term support until the goals are met.

What is High-Fidelity Wraparound Model (Wraparound)?
At the heart of our Wraparound model is you. This person-centered approach is built on compassion, collaboration, and community. Together, we create a supportive team that may include family, friends, mental health professionals, and others you trust. Everyone works together to help you move toward your goals—on your terms.
With your voice leading the way, we’ll develop a personalized plan that reflects your needs, values, and hopes for the future. Your team will meet with you regularly to check in, celebrate your progress, adjust the plan as life evolves, and make sure the right support is always in place.
You're not alone in this journey—your team is here to walk alongside you every step of the way.
Your family may benefit from WrapAround Services if you and/or your child are experiencing a combination of the following:
- Difficulty in school
- Behavioral issues
- Aggressive or violent behaviors
- Difficulty following the rules
- Court involvement and/or court diversion
- Involvement with Child Protective Services
- Mental Health issues
- Drug and/or alcohol involvement
- Difficulty in coordinating multiple services

Step I : Engagement
- Service Coordinator explains the process to the family
- The family shares their concerns, needs, hopes and dreams
- The family identifies the service providers involved with the youth and family
- The family identifies the people that provide natural support :
- Extended family members
- Friends of youth and family
- Community supports like clergy, youth ministers, coaches and other program leaders
- Immediate Crisis Stabilization Plan may be developed as needed
Step II: A Strengths, Needs, Culture, Discovery is completed that includes :
- Youth and family individual strengths and achievements
- Youth and family needs
- Family culture, family beliefs and traditions
- What has worked for the youth and family in the past
- Family vision: what would make you feel the youth is safe and successful
- Input from others involved with youth and family
Step III : Developing a Plan
Your identified team members come together with the youth and family to work with your Wraparound Service Coordinator to :
- Develop a team mission that your team will work on together
- Look at the youth and family strengths
- Look at the youth and family needs and prioritize those needs
- Develop a Plan where several different ways to meet a prioritized need are matched with identified strengths
- Together your team decides what the responsibilities and tasks will be for each member
Step IV : Working the Plan
Your team comes together regularly to review the Plan. As a team you will :
- Review and celebrate accomplishments
- Review the plan to decide if it is working and make changes if needed
- Assign new tasks to team members
- Review prioritized needs and identify the next need to work on as a team

COMMUNITY EDUCATION
|
Our Community, Education, and Prevention (CEP) Program aims to promote awareness and improve mental health of community members here in Wayne and Holmes counties. Prevention and early intervention can reduce the impact of mental health and can help all of us learn useful skills and strategies to improve our overall wellbeing. Our CEP program provides a variety of services, including:
Trainings and Groups
Parenting Education Class
Our “Helping Children Succeed After Parents Separate” is a class for parents going through a divorce or dissolution, that reviews the challenges of separation on both parents and children and discusses helpful coping strategies for families.
QPR Gatekeeper Training
QPR stands for Question, Persuade, and Refer — the 3 simple steps anyone can learn to help save a life from suicide. Our QPR Gatekeeper training teaches participants signs of a suicide crisis and how to question, persuade, and refer someone to help.
Support Groups
Traumatic Brain Injury Support Group. A monthly support group for individuals experiencing emotional, identity, occupational, or relationship difficulties after traumatic brain Injury, stroke, aneurysm, or other damage to the brain.
School-Based Programs
Signs of Suicide (SOS)
Our staff facilitates the SOS program to both middle school and high school students throughout Wayne and Holmes Counties. SOS has been used by thousands of schools for over 20 years. SOS teaches students how to identify depression symptoms, potential suicide risks in themselves or a friend, and how to ACT (Acknowledge, Care, and Tell a Trusted Adult).

COUNSELING & OUTPATIENT THERAPY

At The Counseling Center, we provide counseling and psychotherapy to help you find real solutions to the challenges in your life. By talking with one of our experienced therapists, you can begin to understand your struggles more clearly, develop healthy ways to cope with stress, improve your relationships, reduce mental health symptoms, and feel more confident in yourself and your daily life. We offer individual, family, martial therapy and group therapy.
We provide assessment and treatment for issues such as depression, anxiety, trauma, ADHD, bipolar disorder, relationship problems, and more. We use evidence based practices such as helping people with motivation to change, cognitive-behavioral therapy (CBT), dialectical behavioral therapy (DBT), EMDR, and others depending on the presenting problem and treatment plan that you develop collaboratively with your provider.
What Is Mental Health Therapy What Can It Accomplish?
We all need to keep learning and growing throughout life. When we don’t, we can feel:
- Unhappy or stuck
- Stressed or overwhelmed
- Lonely or disconnected
- Unproductive or unmotivated

Therapy—also called counseling or psychotherapy—can help you move past the barriers holding you back, help reduce mental health symptoms, and increase coping skills and strategies to help you with your presenting concerns

What Can I Expect from My Therapist?
Your therapist will:
- Listen with care and without judgment
- Offer support, guidance, and honest feedback
- Use evidence based treatment strategies personalized to your concerns
- Help you see challenges in new ways
- Suggest healthier ways to cope and communicate
- Sometimes challenge you to grow—this can feel uncomfortable, but it’s often a sign of progress
Setting Goals in Therapy:
- You’ll work with your therapist to set clear, personal goals
- These goals help focus your therapy and track your progress
- You may occasionally fill out a questionnaire to check how things are going
Additional Support Services: If needed, your therapist may suggest other helpful services, such as:
- Crisis support – for help during tough moments between sessions
- Group therapy – to connect with others facing similar challenges
- Case management – to coordinate other care (mental health, medical, housing, etc.)
- Daily support programs – for structure and community during the day
- Job support – to help you find or keep work
- Medication management – Medication can help with symptoms but doesn’t solve deeper issues on its own
Therapy Is Temporary—but the Skills You Learn Last! Your therapist’s main goal is to help you become more confident and independent and over time, you’ll learn how to manage challenges on your own.

How Long Does Therapy Last?
- Most therapy sessions last about one hour
- Sessions usually happen every 1 to 3 weeks, depending on your needs
- Therapy is usually short-term—many people reach their goals in 12 sessions or fewer
- If you need more time, your therapist may recommend continuing for additional sessions
How Can I Make Therapy Successful for Me?
Making the Most of Therapy
Therapy can be a powerful tool for growth, but it doesn’t come with guarantees. The progress you make depends largely on your own effort and commitment. The more you put into the process, the more you’re likely to get out of it. Being honest and open with your therapist about your experiences, thoughts, and feelings is key. It's also important to stay open to new ways of thinking, looking at your challenges, and trying out different approaches to behavior and problem-solving.

INTENSIVE HOME-BASED Treatment (IHBT)
|
Intensive Home-Based Treatment (IHBT) is an in-home mental health program designed to provide youth and families with the support and services needed to address mental health challenges. The program enables youth to remain in their homes, offering the least restrictive and most normal living environment possible. It is delivered by a team of professionals working closely with the family to achieve positive mental health outcomes.
Frequently Asked Questions About Intensive Home-Based Treatment (IHBT)
What Does Home Based Treatment (IHBT) Accomplish?
IHBT Accomplishes the Following:
- Provides in-home mental health services: Youth and families receive therapy and support in their own homes, promoting comfort and familiarity.
- Collaborative treatment approach: A licensed mental health therapist and a qualified case manager work together with the family over a 3-6 month period to meet identified needs and treatment goals.
- Access to additional services: Families have access to mental health providers, psychiatric services, and crisis response as part of the program.
- Focus on risk prevention: IHBT targets mental health issues that may put youth at risk, ensuring early intervention and support.
- Promotes positive youth development: The program works to foster healthy development and coping strategies for youth.
- Enhances family functioning: IHBT strengthens family relationships and provides tools for healthier communication and support within the family unit.
What Can I Expect from My IHBT Team?
Each team is equipped with licensed mental health professionals who utilize evidenced-based approaches to support the youth and family’s needs. Our staff will provide both individual and family counseling sessions using interventions such as Cognitive-Behavioral Therapy, Motivational Interviewing, Dialectical Behavioral Therapy, Solution-focused Techniques, Life Adventure for Families, Trauma-informed care practices, and more.
Our IHBT teams have experience working with:
- Anxiety/Depression
- ADHD
- Autism
- Oppositional Deviant Disorder
- Trauma
- Suicidal/Homicidal Ideations/Attempts
- Youth involved in juvenile court

How Much Time Does IHBT Take?
IHBT is a short-term, intensive program with services ranging from 3 to 6 months. Youth and family will meet with the providers on a weekly basis, including at least 2 sessions per week for a minimum of 3 hours total. Services are held at home or in the community, which can include your child’s school.
How Can I Make IHBT Successful for My Child and Family?
IHBT is a family-based program which requires engagement from both the youth and families. Here are some ways to have a positive IHBT experience:
- Engage in the Process. IHBT is all about growth. Be willing to reflect, try new perspectives, and practice strategies outside of session.
- Be on time and present in sessions. If your family is frequently canceling or missing appointments, your IHBT team may suggest ending services so that they can be available to other families needing their services.
- Be Patient. It's okay to have uncomfortable thoughts or feelings, to feel frustrated and to be tempted to quit, especially in the early stages of IHBT. But the research shows that, if you can push beyond those thoughts and feelings, you and your family can make significant gains.
- Communicate with your team. If something isn’t working or you feel stuck, bring it up. Sharing your thoughts, feelings, and experiences as openly as possible will help you and your family accomplish your treatment goals.

MENTAL HEALTH DAY TREATMENT
|
As individuals, there are times in which we get overwhelmed and our ability to cope with our situations requires more support than typical mental health services. In these situations, entering a more intensive, frequent level of service can meet the needs that other services are not.
Day Treatment can help to assist by providing daily support to an individual in building up their skills and understanding of their current mental health issue. This service includes both individual and group support to allow for additional insight to be built into current stressors and potential interventions to be identified. The goal is to have an individual stable and able to remain in the community and out of psychiatric hospitalization by the end of the program.
ABOUT MENTAL HEALTH DAY TREATMENT
What It Provides:
- Day treatment provides intensive, person-centered and comprehensive care through an initial assessment with a licensed professional to determine individual needs and collaborate on an individualized treatment plan for the program.
- Day treatment runs 5 days a week for a minimum of 3 hours of treatment services per day.
- These services can include both individual and group support and are provided by licensed and/or certified treatment professionals.
Interventions:
Interventions of the day treatment program can include:
- Skills Development,
- Problem solving,
- Conflict Resolution,
- Emotions/Behavior Management,
- Positive Coping Skills development,
- Psychoeducation,
- Community Resource Connection,
- Relational Skill Development
- other personalized interventions as developed collaboratively as a part of the individual treatment plan.
Psychiatric Services:
- ·Psychiatric services are also provided for those who need medication to assist in stabilizing Symptoms.
- The length of treatment will be determined by you and the Day Treatment therapist in the first meeting as a treatment plan is developed.
- Treatment time can vary depending on your level of participation in the process in reaching your personal goals discussed in the treatment planning process.

Orchard Court Apartments
|
Orchard Court Apartments offer a safe, stable place to call home through our Permanent Supportive Housing program. With fifteen one-bedroom units, this program is designed for young adults-typically between the ages of 18 and 26-who are experiencing homelessness and living with a serious and persistent mental illness. At Orchard Court, residents are welcomed into a supportive community where they can begin to build a foundation for independence. Through compassionate case management and Wraparound services, individuals receive the guidance and care they need to develop everyday living skills, strengthen coping strategies, and move forward at their own pace.

PSYCHIATRIC SERVICES
|
Our Psychiatric Services department offers medication management for youth and adults as a key approach to addressing serious symptoms associated with mental health conditions. Clients may be referred by a primary care provider, follow up after discharge from a psychiatric facility, or independently seek out psychiatric services. Today’s medications are highly effective and help many individuals experience significant symptom reduction and improved quality of life, particularly when used alongside other therapeutic treatments.
What Are Psychiatric Services and What Can They Accomplish?
Our psychiatric providers specialize in prescribing and monitoring medications that can help manage serious mental health symptoms. These symptoms often affect how a person thinks, feels, relates to others, and handles stress.
Medications may be helpful in treating a variety of mental health conditions, such as:
- Depression
- Anxiety
- Bipolar disorder
- Schizophrenia
- PTSD
- Other behavioral or emotional disorders
When used appropriately, medications can:
- Reduce distressing or disruptive symptoms
- Improve day-to-day functioning
- Enhance overall quality of life
Medication works best when combined with other supports like:
- Counseling or therapy
- Case management
- Group support
- Other community-based services
Our psychiatric team works closely with you and your therapist to make sure your treatment plan is personalized, effective, and supportive of your long-term well-being.
What Can I Expect from My Provider?
Our psychiatric providers will evaluate your need for medication and prescribe the appropriate treatment when necessary. They will also monitor how well the medications are working for you and make adjustments as needed. Your provider will explain the purpose of the medications and what results to expect.
- It’s important to keep your provider informed about any concerns or side effects you may experience while on medication.
- If you have minor questions or concerns after hours, a prescription voicemail line is available for assistance.
If your provider feels that additional support through counseling or case management would be helpful, they will refer you to a specialist at The Counseling Center. It’s important to follow through with these recommendations to ensure the best care possible.
How Much Time Does This Type of Service Take?
Your initial appointment with the provider will typically last one to one and a half hours.
During this time, the provider will:
- Review your personal and medical history
- Review available records
- Discuss their findings and recommend a treatment plan
After the initial appointment, subsequent visits will usually last 30 minutes. The frequency of follow-up appointments will depend on your treatment needs and the type of medication you're prescribed. As you make progress, appointments may be scheduled further apart at the provider's discretion.
Some patients may require laboratory tests for various reasons as part of their treatment.
To get the best results, it’s important to attend all scheduled appointments. If you need to cancel, please provide 24-hour notice. Multiple missed appointments may lead to medication refill delays or discharge from services.
Telemedicine appointments may be available, but new patients must attend their initial appointment in person at the Wooster main office.
How Can I Make Psychiatric Treatment Successful for Me?
Your provider will work with you on developing an individualized treatment plan. Other recommendations or referrals will be discussed at that time. These recommendations may not be related to medication but are important to your health. Be open-minded to additional suggestions and practices that will contribute to your overall progress.
Keep a journal of your experiences, thoughts, medication side effects, changes/challenges, and questions for your provider. Bring this to your appointment.
- Practice healthy eating, exercise, and sleeping routines.
- Seek positive relationships, social activities, and support groups.
- Always tell your provider about any changes in your health or medication.
- DO NOT USE ALCOHOL OR NON-PRESCRIPTION DRUGS while taking prescribed medications and take any prescribed medications only as directed.
- Call the department during office hours to discuss any concerns with the nurse, if needed, in between appointments. We are here to help you!

Our Residential Services program offers a supportive and caring group home environment for individuals facing significant and persistent mental illness. We are dedicated to providing a safe space where residents can receive the support they need to thrive, develop important life skills, and work toward greater independence. Here, we focus on nurturing each person's well-being and helping them lead fulfilling lives with the care and respect they deserve.

Residential Services offer supportive services for single adults living with significant and persistent mental illness. We provide care through five group homes designed to help residents with daily living activities they may find challenging due to their mental health.

- Medication management
- Building daily routines and life skills
Our homes offer a safe, stable environment with 24/7 staff support. We focus on helping each person gain independence, build confidence, and feel part of a caring community.

RESPITE CARE
|
Welcome to our Respite Program for children and youth. This program is designed to support youth with complex mental health and substance use needs, while also providing care and relief for their families and caregivers. Respite services are available and provided by qualified, trained respite providers that are thoroughly screened and trained in supporting youth with behavioral health concerns. We provide short-term respite for up to a few hours per day, and not overnight respite.

Respite Services are here to provide support while making sure your child is in safe, caring hands. It’s a way to support your family’s well-being and help keep things balanced at home.

Youth Case Management
Raising children to become responsible and fulfilled adults can be challenging, especially when dealing with emotional, behavioral, or school-related issues. For children facing complex, long-standing problems, specialized services and support are essential. What Are Youth Case Management Services?
- Youth Case Management services are provided in home and/or in the community, meeting the youth and family where they are.
- Services focus on addressing youth’s individualized mental health needs, with an emphasis on care coordination and collaboration with community resources.
- The overall focus of Youth Case Management services is to increase the youth’s ability to function successfully in the family and community environments.
What Can I Expect from My Youth Case Manager?
At the beginning of services, your case manager will conduct a thorough assessment to understand the youth and family’s needs, strengths and challenges to living successfully in the community. Based on this, the youth, family, and case manager will work together to develop a comprehensive individualized service plan and develop goals for treatment. This is an ongoing plan that can be updated at anytime to show progress or changes in goals.
Your case manager will focus on different facets of life including but not limited to:
- Help youth build appropriate independent living skills and managing basic needs
- Identify and assist in growing natural supports
- Monitor mental health symptoms and help youth develop effective coping skills
- Education and support for family, specific to youth’s needs
- Link youth/family to community resources and supports
Frequency of Youth Case Manager Services
Youth Case Management services are individualized, and frequency will be determined through medical necessity. Your family and the case manager will work to establish how often to meet. Some months may vary based on individual need. Case Management can be short-term or result in more long-term support until the goals are met. If youth are over the age of 18 and are still in need of services, they may be eligible for Adult Case Management services.
