Important Notice regarding irs form 1095-c
Pursuant to current federal law and applicable Internal Revenue Service (IRS) regulations, employers are no longer required to automatically furnish or mail Form 1095-C (Employer-Provided Health Insurance Offer and Coverage) to employees.
Form 1095-C provides information regarding health coverage offered to eligible employees. While the Employer is not required to automatically distribute this form, a paper copy will be provided to any employee who requests one.
Employees may request a paper copy of Form 1095-C at any time. Upon request, the Employer will furnish the requested form within thirty (30) days of the date the request is received.

This notice is provided in accordance with federal reporting requirements under the Affordable Care Act and is intended to inform employees of their right to request Form 1095-C.
